Exemplar Health Care

Exemplar Health Care is one of the country’s leading nursing care providers for adults living with complex needs arising from brain injuries, dementia, mental health conditions, neuro-disabilities and physical disabilities. The people we support would often be in a hospital or higher acuity setting if not placed with us. Our care enables them to stay in the community and reduces hospital and secure facility admission. We have over 50 care homes across Yorkshire, the Midlands, the North East and the North West of England.

  • Welcome to Exemplar Health Care (1.97 MiB)

Exemplar Health Care is one of the country’s leading nursing care providers for adults living with complex needs. Our Nurse-led teams are trained to support adults living with complex mental health needs, dementia, neuro-disabilities including brain injuries, Huntington’s disease and Parkinson’s disease, and physical disabilities including cerebral palsy, spinal injuries and musculoskeletal disorders. Read more about our complex care services.

  • FAQs about our complex care services - for commissioners (0.86 MiB)

Exemplar Health Care homes aren’t standard residential/elderly care or supported living services. The acuity of our services sits just beneath a hospital setting, meaning the needs of the people we support are often ‘too complex’ for mainstream care providers. This document answers some of the common questions that we get asked.
